Each policy defines covered events specifically. In general, a covered heart attack requires a physician's diagnosis supported by evidence such as EKG changes and elevated cardiac enzymes; a covered stroke typically requires confirmed neurological damage lasting longer than 24 hours. Mini-strokes (TIAs) and angina are usually treated differently and may or may not be covered.
Once a completed claim and the required physician documentation are submitted, most carriers pay the lump sum within a few weeks. It's paid directly to you in one lump sum — not in installments and not to a hospital or doctor.
Yes. It's supplemental coverage that pays in addition to Medicare, Medicare Advantage, a Medicare Supplement, employer coverage, or an ACA plan. The benefit goes to you, so it doesn't reduce what your primary insurance pays.
